BASEBALL CLINIC WAIVER

I give permission for my child(ren) to participate in the Battle Jacks Baseball Camp and related activities (the “Camp”) operated by Battle Jacks Baseball LLC and Southpaw Ventures Corp. (“Battle Jacks”).

I understand that participation in baseball and related athletic activities involves inherent risks, including but not limited to physical injury, illness, disability, or other unforeseen incidents. I acknowledge that it is impossible to eliminate all risks associated with participation.

I certify that my child(ren) is/are physically able to participate in the Camp and has/have no medical condition that would make participation unsafe. I authorize the Battle Jacks staff, coaches, employees, volunteers, trainers, or representatives to obtain and/or provide emergency medical treatment for my child(ren) if deemed necessary during participation in the Camp. I understand and agree that I am solely responsible for any medical expenses incurred.

By registering my child(ren) for the Camp, I voluntarily assume all risks associated with participation and, on behalf of myself and my child(ren), release and forever discharge Battle Jacks Baseball LLC, Southpaw Ventures Corp., MCCU Field at Bailey Park, their officers, employees, coaches, agents, volunteers, contractors, sponsors, affiliates, and representatives from any and all claims, liabilities, demands, damages, costs, or causes of action arising out of or related to participation in the Camp, including those resulting from negligence to the fullest extent permitted by law.

I further agree not to bring any legal action against the Battle Jacks or any affiliated parties for injuries, damages, losses, or claims arising from participation in the Camp.

I also grant permission for the Battle Jacks to use photographs and/or video taken of my child(ren) during Camp activities for promotional, marketing, social media, and advertising purposes without compensation or further approval.

By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Waiver and Release of Liability.

Note: By registering for a Battle Jacks baseball clinic or camp, you grant permission for the participant, parent/guardian, and any member of your party entering MCCU Field at Bailey Park to be photographed, filmed, or recorded. These images and recordings may be used at the sole discretion of the Battle Creek Battle Jacks, Southpaw Ventures Corp., and the Northwoods League for promotional, marketing, social media, broadcast, and advertising purposes without compensation or additional approval.
