1) My child will be working in an environment where welding machines, tools, and industrial equipment are in use and will be using these tools to build art bikes.

2) My child will be working in an artist's warehouse environment that may be hot or cold at times.

3) My child's work will be supervised by Art 120 staff, the teachers, or adult volunteers. I believe that my child is mature enough to behave appropriately as a student of this workshop.

4) The completed art bikes will remain the property of Art 120 who will manage their public display. My child's participation will be recognized whenever their bike project is exhibited.

I release Art 120, its sponsors, employees, board members, volunteers and agents from any and all claims and liabilities (including costs and attorney fees) arising out of or in any way connected to my child's participation in the art bike workshop. I further agree to indemnify and hold Art 120 harmless from any and all claims arising from my conduct or the conduct of my child while he or she is a particpant in any Art 120 workshops or future activities.
Yo libero a Art 120, sus patrocinadores, empleados, miembros de la junta directiva, voluntarios y agentes de cualquier reclamación y responsabilidad (incluyendo costes y honorarios de abogados) que surjan o estén relacionados de cualquier manera con la participación de mi hijo en el taller de Art Bike.
