Decertification petition card
I am an Adult Foster Home (AFH) Provider as defined in Section 2.2 of the 2025-2026 Collective Bargaining Agreement between the State of Oregon and the Service Employees International Union (SEIU) Local 503. I no longer desire to be represented by SEIU Local 503, and desire instead to be exclusively represented by the Oregon Care Home Council for purposes of collective bargaining. I understand that my signature may be used to petition the Employment Relations Board to conduct an election to change the exclusive representative of this bargaining unit.

The Oregon Care Home Council has hundreds of provider supporters who already submitted their cards. One industry, one voice. We all stand together for a better future in our care homes. Please fill out the card and be part of the decertification process. It's time for care homes to be in charge of their own future.
