I-9 Eligibility Form

I9 form - MUST be completed by ALL NEW Coaches. I9 is a paper document that requires validation of documentation (preferably a current US passport) by Craig Forsyth or Doug Haw. It is recommended that an appointment be made to do this with Craig in the AAFSC Office to deliver signed form and validate documents. Due within 3 days of first day of work or July 1, 2026 whichever is later.

If you are a NEW COACH (not currently on our 2025/26 staff). You NEED TO COMPLETE ALL OF THE DOCUMENTS AND UPLOAD THEM BELOW

If you are a current coach. Please upload your Green Light compliance screen shot(s) and insurance. YOU are responsible for contacting us for a renweal of documents if your address changes, ect.

Save all files as individual pdfs. Be sure to follow the naming instructions below the upload button. Your files need to be labeled correctly in order to be accepted.
