I grant permission to the Oklahoma International Dance Festival (OIDF) to photograph, video record, and use images or recordings of the student named above for promotional, educational, archival, website, print, and social media purposes without compensation.
I certify that I am the legal parent or guardian of the participant listed above. I understand that participation in dance training and physical activity involves inherent risks, including but not limited to physical injury, illness, accidents, or other unforeseen circumstances. By registering for the OIDF Youth Dance Intensive, I voluntarily assume all risks associated with participation and certify that the student is physically able to participate in dance activities. I hereby release and hold harmless the Oklahoma International Dance Festival (OIDF), University of Central Oklahoma, program faculty, staff, volunteers, directors, representatives, and affiliated organizations from any and all claims, liabilities, damages, injuries, losses, or expenses arising from participation in the program, including transportation to and from program activities. In the event of a medical emergency, I authorize OIDF staff to obtain emergency medical treatment for the student if I cannot be reached immediately. I acknowledge that OIDF reserves the right to dismiss any participant whose behavior is disruptive, unsafe, or inappropriate without refund.
Students are expected to participate respectfully and maintain a positive learning environment for all participants, faculty, and staff.
