I hereby grant permission to Eagle Performance Academy to record and use photographs, video and audio recordings ("media") of myself or my likeness for the purpose of assessing skills, promoting and documenting Eagle Performance Academy programs, events and activities. I understand that media may be captured during sessions, tournaments or other events associated with Eagle Performance Academy. I hereby authorize Eagle Performance Academy to use such media on its social media platforms (including but not limited to Facebook, Instagram, TikTok, WhatsApp) and in official printed publications, including newspapers, without requiring any further approval, consideration or compensation. I hereby acknowledge that once images or recordings are published online, they may be accessible to the public and subject to downloading or sharing by third parties beyond the control of Eagle Performance Academy.

I hereby confirm that I am physically fit to participate in any session or program and understand that all exercise and use of the facilities are undertaken at my own risk. I hereby release and hold harmless Eagle Performance Academy and The Meridian School, its employees, agents, assignees and representatives from any and all claims, demands, injuries, damages or legal actions arising out of or resulting from my participation in sessions, programs, or use of the facilities. This includes, but is not limited to, any loss or damage to my personal property, motor vehicle or any personal injury, whether caused by negligence or otherwise.

CANCELLATION POLICY
• Sessions must be canceled at least 12 hours in advance.
• Late cancellations and no-shows will be charged in full.
• Medical emergencies will be considered on a case-by-case basis.
• Weather-related adjustments will be communicated promptly.
