Practice t-shirts must be worn and can be purchased for $25. Please select your child's size from the options below. Note the t-shirts are youth unisex sizes.

Batons are purchased for $35. To measure appropriate baton length, measure your child's arm from armpit to the tip of the middle finger and then add one inch. Enter the number below (in inches).

Cancellation & Refund Policy

Cancellations made one month or more before the program start date are eligible for a 50% refund of the program fee of $160.00. Batons may be refunded in full provided the baton is returned to Lakeview Baton Twirling Academy in good condition. T-shirts will not be refunded. Cancellations received less than one month before the program start date are not eligible for a refund. Missed or cancelled classes are non-refundable. Classes may be cancelled at the discretion of Lakeview Baton Twirling Academy in the event of inclimate weather, emergency, or illness. Every reasonable effort will be made to avoid class cancellation.

Liability Waiver

A Liability Waiver will be sent separately and must be reviewed, acknowledged, and signed in order for enrollment to be considered finalized.

Enrollment Terms

Enrollment occurs when this form, payment in full, and any subsequent documentation, e.g., Liability Waiver, have been completed. Spots will be available on a first come first serve basis, and will be limited. If all documentation and payment are not completed by their due dates, the child will not be considered enrolled in the program.

If the email address of your e-transfer is different than the Parent/Guardian Email provided on this form, you must include the Parent/Guardian name and email in the notes section of the e-transfer.

Program: $160.00

Practice tee: $25.00

**Note if you chose to order additional practice t-shirts, the amount ($25.00 per shirt) must be included on the e-transfer.
