First Credential Scholarship Program Proof of Enrollment Form

This form is for students who were previously awarded the First Credenital scholarship and an approved deferment from USHE.

Upon your return, please complete the following form so the scholarship office can assist you. Once USHE has received the proper documentation, the funding and eligibility information will be sent to your chosen institution.

Proof of enrollment after deferment must be submitted by the deadlines listed below:

For Fall Semester – September 15
For Spring/Winter Semester – February 1
For Summer Semester – June 1

For this renewal form, you will need the following:

Your unofficial transcript that includes the number of credits you are enrolled in for your first semester back after deferment (ex. Fall 2027), as well as your name, the name of your college, and your college student ID number. You will upload your document here. Please use this document (scroll to page 2) or contact scholarships@ushe.edu if you need help locating an acceptable document.

Students may receive both the First Credential Scholarship and the Opportunity Scholarship. The First Credential Scholarship shall be applied to tuition and fees first before an Opportunity Scholarship may be awarded.

(Students that have been awarded in 2026 may receive a one-time payment of $625, subject to available funding, to be used at an eligible institution.)
