Commission Request Checklist
Please check the following boxes to confirm that:
All purchased equipment is on-site and fully installed as required (Dimmer/Relay Racks, Power Packs, Stations, Sensors, etc) according to the latest Bill of Materials.
Any missing or damaged items are noted on the Onsite Service Request Form.
All equipment rooms are clean and free of any additional construction within the rooms.
All racks and rack vents are cleared of debris and dust.
Arrangements have been made to provide access to ALL system equipment. (Including ladders, lifts w/ operator if required, etc). Please ensure special safety equipment is available to the tech if needed (harness, special protection equipment, etc).
An EC representative will be available, at all times on-site, to power up/down equipment as needed during commissioning.
EC will coordinate with other trades to ensure all site personnel are aware and prepared for lights to be turning ON/OFF during commissioning.
All system inter-wiring is complete between dimmer/relay racks and control stations as per factory drawings and has been checked for accuracy, shorts, and continuity.
All control wiring is of proper factory specified wiring and all wiring has been certified (as required) and lengths verified.
Red-lined drawings are completed and on-site showing exact wiring route if altered from Factory drawings.
All load lamps are installed if present.
All load circuits have been tested for correct wiring and are free of any shorts.
ALL specific programming information is attached to this document including drawing notes, Sequence of Operations, etc. Someone with decision making authority will be available to verify equipment configuration during commissioning.
If Bacnet system to be tied into equipment ensure all integration personnel are on site and all integrating information (IP Addresses, Bacnet numbers, etc) is attached to this document.
ALL Necessary owners, representatives, and staff will be present for demonstrating and training during the commissioning at the scheduled time.
I HEREBY CERTIFY THAT THE ABOVE CONDITIONS ARE MET AND THE INSTALLATION IS COMPLETE. I UNDERSTAND THAT A PURCHASE ORDER WILL BE REQUIRED FOR ADDITIONAL VISITS RESULTING FROM AN INCOMPLETE INSTALLATION. UNSIGNED FORMS NOT ACCEPTED
