  • SECTION 4: PROGRAM DESCRIPTION

  • The Launch Volleyball Academy June Technical Lab is a purpose-driven, skills-first training program focused on strong mechanics, movement efficiency, and game understanding. Athletes develop confidence and consistency through high-repetition training, individualized feedback, and applied gameplay in a focused, high-quality environment.

    Girls 15-17 | Mondays - June 1, 8, 15, 22

    Boys 15-17 | Wednesdays - June 3, 10, 17, 24

  • SECTION 5: TUITION AND PAYMENT INFORMATION

  • Monthly Payment: $195

    Tuition covers coaching, gym rental, equipment usage, and training operations, and payments are due on the first Monday of the program month. Prorated payments may be considered for athletes starting after the 1st of the month.

    Payments are non-refundable; however, if a session is canceled by Launch Volleyball Academy, a credit may be applied toward a future session or program. No refunds or credits will be issued for missed sessions.

  • SECTION 6: CODE OF CONDUCT

  • Athletes are expected to:

    - Follow coach instructions

    - Demonstrate respect for teammates, coaches, oƯicials, and facilities

    - Maintain a positive, coachable attitude

    Parents/guardians agree to:

    - Communicate respectfully with staff

    - Support program policies and decisions

    - Address concerns directly with the Director

    Launch Volleyball Academy reserves the right to dismiss an athlete for conduct that is unsafe, disruptive, or inconsistent with program values. Tuition will not be refunded in such cases

  • SECTION 7: WAIVER, RELEASE, & ASSUMPTION OF RISK

  • I, the undersigned parent or legal guardian of the above-named minor (“Participant”),acknowledge that participation in volleyball activities, including but not limited toevaluations, practices, training sessions, conditioning, scrimmages, competitions, travel,and related activities (collectively, the “Activities”), involves inherent risks. These risks include, but are not limited to, physical injury, permanent disability, paralysis, illness,emotional distress, property damage, and death.

    I knowingly and voluntarily allow the Participant to engage in the Activities conducted byLaunch Volleyball Academy (“the Academy”). I understand that these risks may arise from, among other things, physical exertion, contact with other participants, use of equipment, facility conditions, environmental factors, or the actions or inactions of other participants,coaches, volunteers, officials, or spectators.

    Assumption of Risk

    I knowingly, freely, and expressly assume all risks, both known and unknown, foreseeableand unforeseeable, associated with the Participant’s participation in the Activities, including risks that may arise from the negligence of the Released Parties (as defined below), to the fullest extent permitted by law.

    Release and Waiver of Liability

    In consideration of the Participant being allowed to participate in the Activities, I, on behalfof myself, the Participant, and our respective heirs, assigns, personal representatives, andnext of kin, hereby release, waive, discharge, and covenant not to sue:

    Launch Volleyball Academy, its directors, officers, coaches, assistant coaches, volunteers,staff, agents, sponsors, aƯiliated organizations, facility owners/operators, landlords, andevent organizers (collectively, the “Released Parties”) from any and all claims, demands, actions, causes of action, liabilities, damages, losses, costs, or expenses of any kind, whether arising from negligence or otherwise, related to orresulting from the Participant’s participation in the Activities, including but not limited toclaims for personal injury, catastrophic injury, permanent disability, illness, death, or property damage.

    Indemnification and Hold Harmless

    I agree to indemnify, defend, and hold harmless the Released Parties from and against anyand all claims, liabilities, damages, losses, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to the Participant’s participation in the Activities, including claims brought by or on behalf of the Participant or any third party.

     

  • SECTION 8: PHOTO & VIDEO RELEASE

  • I grant permission to Launch Volleyball Academy to photograph, videotape, or otherwise recordthe Participant during Activities. I understand these images may be used for promotional,educational, or marketing purposes in any media, without compensation.

  • SECTION 9: ACKNOWLEDGEMENT & AGREEMENT

  • By signing below, the parent/guardian confirms that they:

    - Have read and understand this Agreement

    - Agree to the developmental training program

    - Agree to the tuition and payment schedule

    - Agree to the Code of Conduct

    - Accept the assumption of risk and release of liability

    - Grant permission to photograph or record the Participant during Activities
