I grant to Kohl Meyer Basketball, and its representatives the right to take photographs/videos of my minor child in connection with Kohl Meyer Basketball. I authorize Kohl Meyer Basketball, and its assigns and transferees to copyright, use and publish the same in print and/or electronically. Waiver of Liability and Assumption of Risk

I (hereafter “Participant,” named below) wish to participate in sporting activities, at Kohl Meyer Basketball Camp. I understand that there is a risk of personal injury in the course of instruction and such sporting activities and, with this knowledge, I agree on my own behalf to assume the risk of any injury and damages
during the course of such activities. I understand and agree that this waiver is granted in exchange for access to such participation, instruction, and sporting activities.

Specifically, I, agree to hold harmless Kohl Meyer (hereafter “Releasees), and all other individuals (including their heirs, assigns and representatives), members,
owners, officers, affiliated organizations, sponsors, instructors, employees, and staff connected with any of the above-identified Releasees from all losses, damages, injuries, causes of actions, claims, or complaints in the
event that I am damaged or injured in any way during, participation, competition, instruction and/or performance of any activity whatsoever associated with the Releasees.

Participant agrees to strictly obey staff, coaches, instructors, and to observe all safety rules.

Because of the physical demands of sporting activities including Basketball, I understand that I must be in adequate physical condition to participate in such activities. I understand that I should speak with a health care professional before allowing Participant to begin any sporting program. I understand that in the event of injury, the only medical treatment the Releasees will provide is basic first aid.

By signing, you have read and understand the above Waiver of Liability and Assumption of Risk for your registered participant.

**Important Details**

After submitting registration for your child, you will be redirected to a payment link. Please complete the payment of $125.00 for your registration to be complete. If payment is not recieved, your child will not be registered for the camp.

Refunds will not be given if canceled after June 26th, 2026. Thank you for your understanding.
