MEDIA CONSENT | Do you give JAYU, DesignTO and its employees, agents and assigns permission to photograph, videotape, make an audiotape of, film, record the online program modules, and/or interview me, and to publish said photographs, videotapes, audiotapes, films and/or interviews in whole or in part, separately or in conjunction with other photographs, videotapes, audiotapes, films and/or interviews, in JAYU and DesignTO publications/materials, including (but not limited to) marketing and promotional materials, fundraising appeals, presentations and JAYU’s and DesignTO’s official websites, for current and future use and/or to provide said photographs, videotapes, audiotapes, films and/or interviews to media covering JAYU and DesignTO and to use my name in connection therewith.

MEDIA CONSENT | If yes, you understand that JAYU and DesignTO are charitable arts organizations, and that their publications, presentations and use of said photographs, videotapes, audiotapes, films and/or interviews are for public distribution. I understand further that the photographs (including negatives), videotapes, audiotapes, films and/or interviews shall constitute the exclusive property of JAYU and DesignTO and may be reproduced by JAYU, DesignTO and anyone it has authorized, without compensation or payment to the individual(s) concerned or any other person. I hereby release JAYU, DesignTO and its employees, agents and assigns from all and any claims that may arise from the use of the photographs, videotapes, audiotapes, films and/or interviews.

I/we represent and warrant that the exhibited works will not infringe on the intellectual property rights of any third party. I/we shall indemnify, defend, and hold harmless JAYU, DesignTO, Toronto Design Offsite Festival and its officers, directors, members, employees and agents from and against all direct or indirect claims, actions, damages, losses or expenses, including reasonable legal fees and disbursements, incurred in connection with any third-party intellectual property claim, action, or proceeding.
