• 2026 Wag-O-Ween - RESCUE REGISTRATION FORM

  • Format: (000) 000-0000.
  • Would you like to participate in the raffle? We will also have a separate silent auction, raffles will be given out throughout the day of the event. If you select "yes" we will contact you closer to the event date.
  • IMPORTANT INFORMATION REGARDING SET-UP AND BREAKDOWN:

    • Vendor/Rescue Space is 10x10 without electricity
    • Vendors/Rescues must provide their own tent, tables, chairs, etc.
    • Set up begins at 9:00 am
    • No breakdown until 3:00 pm
    • This event will happen, rain, wind, cold, or hot, please plan accordingly
    • No adoptions may take place at the event, but adoption applications can be collected
  • AGREEMENT
    By participating in Wag‑O‑Ween, the rescue registering agrees:

    All animals are healthy, vaccinated as required, and safely restrained at all times
    The organization is responsible for the behavior and care of all animals in their control
    US Dog Coalition & Rescue is not liable for injury, loss, or damage involving animals, people, or property
    The organization agrees to follow all event rules and applicable laws
    The organization will keep their area clean and remove any waste
    The organization allows photos/videos taken at the event to be used for promotional purposes
    US Dog Coalition & Rescue reserves the right to remove any participant for safety or rule violations.
