  • WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND MEDICAL CONSENT

    1. Assumption of Risk: I, the undersigned, am the parent or legal guardian of the child named above. I recognize that participation in baseball camps, tryouts, and related activities involves inherent risks of physical injury, including but not limited to those caused by thrown or batted balls, bats, field conditions, and physical contact with other players. I voluntarily assume all risks associated with my child's participation in the Bama Bomb Squad tryout/camp.

    2. Release of Liability: In consideration of my child being permitted to participate, I hereby release, waive, discharge, and covenant not to sue the Bama Bomb Squad, its coaches, organizers, volunteers, facility owners, and affiliates (collectively, the "Releasees") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to me, while participating in these activities, whether caused by the ordinary negligence of the Releasees or otherwise, to the fullest extent permitted by Alabama law.

    3. Medical Emergency Authorization: In the event of an emergency where I cannot be reached, I grant permission to the coaching staff and organizers to obtain necessary medical care and treatment for my child, including first aid, CPR, and transport to a medical facility. I agree to assume all costs associated with such medical treatment.

    4. Media Release: I grant permission to the team and its representatives to photograph and/or video record my child during the tryout/camp. I authorize the use of these images or recordings for team communications, evaluation, live-streaming, and promotional materials without compensation.

    Acknowledgment: By checking the box below, I acknowledge that I have read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I check it freely and voluntarily without any inducement.
