Permanent Eyebrows Makeup Terms & Conditions Agreement D Brows Studio-pmu Client Agreement & Consent. ■By signing this agreement; ●I confirm that I voluntarily consent to receive permanent makeup services from D Brows Studio. ●I understand and agree to the following terms and conditions. 1. Results & Expectations. ●Permanent makeup is a cosmetic tattoo procedure and healed results vary from person to person. ●No specific shape, color, or result can be guaranteed. ● Multiple sessions and touch-ups may be required to achieve desired results. ●Final healed results depend on skin type, lifestyle, medical conditions, and aftercare. 2. Touch-Ups & Additional Appointments. ● A perfecting touch-up is usually recommended 4–8 weeks after the initial procedure. ● Any additional touch-ups after the recommended period may be charged separately. ● Failure to attend the touch-up within the recommended timeframe may result in extra fees. 3. Pre-Appointment Requirements. ● I understand that I should avoid the following before my appointment: ● Alcohol for 24 hours. ● Excess caffeine on the day of appointment. ● Botox or fillers within 4 weeks. ● Retinol, acids, or harsh skincare products near the brow area. ● Excessive sun exposure or chemical peels. 4. Aftercare Responsibilities. ● I agree to follow all aftercare instructions provided by the artist. ● I understand that failure to follow aftercare instructions may affect healing and final results. ● I understand that itching, scabbing, dryness, tenderness, and fading during healing are normal. 5. Medical Disclosure. ● I confirm that I have disclosed all medical conditions, allergies, medications, and skin sensitivities honestly. ● I understand that withholding medical information may increase risks or affect results. 6. Risks & Liability. ●I understand that although proper care and sanitation procedures are used, possible risks include: ● Allergic reactions. ● Infection. ● Uneven healing. ● Dissatisfaction with shape or color. ● Pigment fading or migrationI release D Brows Studio and the artist from liability for complications resulting from: ● Failure to follow aftercare instructions. ● Undisclosed medical conditions. ● Allergic reactions or unforeseen skin responses. 7. Deposits & Cancellation Policy. ●Deposits are non-refundable and secure the appointment date and time. ● Rescheduling should be done at least 24–48 hours before the appointment. ● Late arrivals may result in shortened treatment time or cancellation. ● No-shows may lose their deposit and require a new booking fee.
