  • Below is your entry form for the South Carolina Aggregates Association Safety Awards Program. The award recognizes your company's overall safety record for each individual plant in South Carolina for the 2025 calendar year.

     

    Awards for individual stationery and portable plants are based on no lost time injuriesin 2025. Lost time injuries are defined as injuries to non-office employees that result in permanent total or permanent partial disability and/or days away from work.

     

    To be considered for a 2025 SCAA Safety Award, please complete the form by June 30, 2026.

     

    Award recipients will receive a plaque that can be proudly displayed in the main office as well as a 3 ft by 5 ft banner that can be displayed at the entrance of the award-winning facility(s

     

    There will be a $100 application fee for each plant submission, which includes one plaque and one banner, and a $50 application fee for each individual miner submission, which includes one plaque. If additional banners are requested for multiple locations, please include an additional $50 for each additional banner. Members will be invoiced after submission.

     

    Awards will be presented at the 2026 SCAA Workshop and Exhibit.

  • Plant Safety Awards

  • In the space below, please list all separate plants operational in 2025 including the total number of hours worked for the year and the date since that site's last lost time injury. Please print all information and use a separate sheet if necessary. Even if you had recordable injuries, please fill out this form as the SCAA uses this information to compile statewide frequency rates.

     

    (a) Indicate MSHA reportable hours for the following subunit codes (03 for our regular surface quarries, 06 for sand & gravel dredging operations, 30 for production plants (crushing/screening operations), 99 for scale-house operations Do not include such activities as concrete and bituminous plants, block plants, etc. (b) Recordable injuries as reported on MSHA Form 7000-1 and in accordance with 30 CFR Part 50. (c) Lost-time accidents and workdays charged are those prescribed by MSHA regulations.

  • Individual safety awards are based on 15 years, 20 years and 25 years length of time with no lost time injuries. Lost time injuries are defined as injuries to non-office employees that result in permanent total or permanent partial disability and/or days away from work.
