AGREEMENT TERMS — READ CAREFULLY
In consideration of being permitted to participate in the above Eagle OPS Foundation Rally Point or activity (the 'Event') held on the above private property (the 'Property'), I, the undersigned Participant, on behalf of myself, my heirs, executors, administrators, and assigns, agree to the following terms: 1. ASSUMPTION OF RISK. I understand that participation involves inherent risks of property damage, personal injury, illness, or death. I voluntarily assume all such risks, known and unknown. 2. RELEASE AND WAIVER OF LIABILITY. I release, waive, and discharge Eagle OPS Foundation, its directors, officers, employees, volunteers, agents, and the Property Owner/Host (the 'Released Parties') from all liability and claims arising out of my participation, whether caused by negligence or otherwise, to the fullest extent permitted by Oklahoma law. 3. INDEMNIFICATION OF PROPERTY OWNER. I agree to indemnify and hold harmless the Property Owner/Host from any claim, loss, or expense arising from my participation or presence on the Property. 4. MEDICAL AUTHORIZATION. I authorize Eagle OPS representatives to secure emergency medical treatment for me. I am responsible for all costs. 5. PHOTO, VIDEO, AND MEDIA RELEASE. I grant Eagle OPS Foundation the irrevocable right to use my name, image, voice, and likeness from the Event for any lawful purpose including marketing, fundraising, social media, and grant reporting. I receive no compensation. 6. CONDUCT. I agree to follow all reasonable instructions of Eagle OPS staff and the Property Owner/Host and to refrain from unlawful or unsafe behavior. 7. FIREARMS, ALCOHOL, AND CONTROLLED SUBSTANCES. I will comply with all applicable laws and will not participate while impaired. 8. GOVERNING LAW. This Agreement is governed by Oklahoma law, with venue exclusively in Tulsa County, Oklahoma. 9. SEVERABILITY. If any provision is invalid, the remaining provisions remain in effect. 10. ENTIRE AGREEMENT. This contains the entire understanding between the parties on this matter.
