WCCSD Summer Skate Camp Registration
Registration Process
To secure your child’s spot, please complete this registration at least 1 week prior to the camp date(s) selected. Payment must be received in full prior to beginning of camp in order for the camper to be able to attend. Skate Camp is limited to a maximum amount of 20 campers per camp week. Secure your place ASAP! All required information, including waiver on the bottom of this form must be fully completed before submission.
Skate Camp will be held at the Willow Creek skate park. Beginner, intermediate, or advanced - all skaters are welcome and encouraged to join the fun! Instructed by local, skilled skaters, skate campers will learn park etiquette, street and vert riding and of course... STLYE! Sorry, skate camp is for skateboards only. BMX bikes, scooters, and other wheels are NOT permitted. Helmet and full pads must be worn during all skateboarding participation.
Ages: 6-13 years.
Days/Times: Monday - Friday, 8 a.m. to noon. Early drop off available at 7:30 a.m.
Weekly Fee: $115 for in district participants or $125 for out of district participants. All campers are responsible for bringing their own lunch daily and their own water bottle. We will have plenty of water to keep campers well hydrated. All campers will receive an awesome/limited edition skate camp tee reserved only for skate camp attendees!

Liability Waiver & Release
I hereby agree and declare that I am the legal parent/guardian of the above-named child and hereby consent to the child's participation in skate activities in the Willow Creek Skate Camp, hosted by the Willow Creek Community Services District. I understand that activities of the kind described may result in physical injury to my child, but nonetheless, I specifically request that he or she be allowed to participate in those activities. If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s). I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer skate camp activities at the Willow Creek skate park and I consent to the participation of the above-named child to these activities. The Willow Creek Community Services District can not be held responsible for any injury that may occur to the child in the duration of this skate camp.
