*Eligible pets must be your own animals, living in your own household. Pets of "family and friends" are not eligible for your employee discount. Improper use of the employee discount may result in disciplinary action up to and including termination.
Changes to this list may be made only under the following circumstances:
- New Pets - If you purchase or adopt a new pet and do not already have four pets listed, you may add another at any time. (Please understand that fostered pets are not eligible for the employee discount.)
- Deceased Pets - If one of your 4 pet designated pets is deceased, you may add another. Unless, the deceased pet is one of 4 pets receiving a discount in that calendar year.
Hospital employees: To use your Employee Discount, you must submit this form to your Hospital Manager at the beginning of each year or if qualifying changes are needed.
Non-hospital employees: To use your Employee Discount, you must submit this form to your HR department at the beginning of each year or if qualifying changes are needed.
