Registration & Payment Policy:

・Deposit: $100 (required to secure registration) An additional $50 deposit is required for Medium or Large backpack orders.
・Final Balance Due: August 31, 2026
・All payments are final and non-refundable.
・Payment must be received within 24 hours of registration to avoid automatic cancellation.

Please submit a separate registration for each child. Multiple children cannot be registered on a single form.

Terms & Conditions
By submitting this registration form, you agree to the following terms:
1. Deposits & Securing Costumes: A $100 non-refundable deposit is due at registration. Medium and Large Backpack options require an additional $50 non-refundable deposit. Costume selections are not secured until all required deposits are received. All payments are non-refundable.
2. Final Payment Deadline: The final remaining balance must be paid in full by Monday, August 31, 2026.
3. No Changes or Switches: Once a section and costume size/style have been selected and submitted, no changes, switches, or modifications will be permitted.
4. Parental Responsibility: Parents/Guardians confirm that all information, measurements, and details submitted are 100% accurate. You accept full financial responsibility for all payments associated with your child’s costume.
5. Media Release: You grant D’Midas Intl NY, the section leaders, designers, representatives, employees, and agents permission to photograph and video record your child during carnival events. This media may be used for promotional, marketing, website, social media, and future advertising purposes without compensation.
