Please call the office immediately if any of the following reportable incidents occur with the person you support:

• Abuse, Neglect, Mistreatment, and/or Exploitation
• ER Visit
• Hospital Admission
• Falls
• Injury that requires medical attention
• Seizure (IF the person does not have a diagnosis of a Seizure Disorder)
• Behavior with physical aggression
• Fire in person's home
• Severe weather damage to the person's home
• Police or Department of Human Resources involvement at the home
• Elopement
