I understand this is a model-priced service intended for content creation and portfolio purposes. Submission of this application does not guarantee selection, and one winner will be contacted on the 15th of each month. If selected, I understand a $40 nonrefundable deposit is required to secure my appointment and will go toward the total cost of the model set. I understand I will have 24 hours to book after being contacted and that my appointment must be scheduled within 2 weeks of selection. Failure to meet these booking requirements may result in forfeiture of the opportunity and ineligibility for future model opportunities. I understand standard cancellation, rescheduling, late arrival, and no-show policies still apply after booking, and that additional consent forms, appointment policies, and service information will be provided at the time of booking. I confirm I am available anywhere between Monday-Friday 10am-3pm, am 18 years or older, am a new client, currently do not have lash extensions or previous lash work on my lashes, and have healthy natural lashes with no known allergies or sensitivities to lash adhesive. I understand results may vary based on my natural lash health and anatomy, and that all model sets are fully customized based on Taylor’s professional artistry, discretion, and my natural lashes. I agree to arrive with clean lashes free of makeup, mascara, and oils. I understand appointments may take longer than a standard appointment due to content creation (about 3 hours). I agree to being photographed and/or filmed for Earthbound Beauty marketing, portfolio, website, email, and social media content, including before and after photos/videos. I understand these images and videos may be used for promotional purposes without compensation. I also agree to receive occasional email and text updates regarding specials, openings, and future model opportunities.
