OFA Youth Soccer (Ages 3-4)

A fun, beginner-friendly soccer program designed for ages 3–4, focused on movement, coordination, and early skill development in a positive environment.

Time: 9:00–9:45 AM OR 10:00-10:45AM

📍 Location: Bobby Bonds Park

💲 Price: $128

Each 45-minute session is led by OFA trainers using fun, structured activities that introduce young players to the game in an encouraging environment.

Players will develop:
• Coordination and balance
• Ball control basics
• Confidence
• Social skills

🎽 Included:
• Soccer training shirt
• End-of-program certificate

👟 Required:
• Shin guards
• Cleats or athletic shoes
• Water bottle

(All soccer equipment will be provided.)

👨‍👩‍👧 Parent Participation:
Parent participation may be needed depending on the child’s comfort level, attention span, or readiness to participate independently. Our goal is to create a positive and supportive first soccer experience for every player.

I, the parent/guardian of the participant, a minor, agree that the Participant and I will abide by the rules of Norberto Ochoa Orejel and Ochoa Football Academy LLC. I understand that there is risk associated with playing soccer, all sports and field related activities. In consideration, for any participated activities with trainer Norberto Ochoa Orejel or associated personnel and volunteers of the Ochoa Football Academy LLC, my signature indicates that I assume the risk of any injuries that my child may sustain while participating with the Ochoa Football Academy LLC trainings/clinics (Norberto Ochoa Orejel).

 

I consent that Ochoa Football Academy LLC (Norberto Ochoa Orejel) shall have the absolute right and permission to publish any and all photographic portraits, videos taken of the Participant in publications, online, social media platforms and in other communications related to the mission of Ochoa Football Academy LLC without compensation or approval rights.

 

I, the undersigned, have carefully read and understood this agreement and all of its terms. I understand that this is a release of liability which will legally prevent me or any other person from filing suit or making any other legal claim for any injury.  I nevertheless enter into this agreement freely and voluntarily and agree that it is binding upon me, my heirs, assigns and legal representatives.

 

By signing this waiver and release of liability form, I acknowledge that I HAVE READ AND FULLY UNDERSTAND AND AGREE TO ALL OF ITS TERMS AND CONDITIONS. I further state that I have executed this waiver and release of liability form voluntarily and with full knowledge of its significance.

 

PARENTS AND GUARDIANS (if participant is under 18 years of age) 

If the participant is a minor, the parent/guardian agrees to assume all liability and to HOLD HARMLESS AND INDEMNIFY Norberto Ochoa Orejel and Ochoa Football Academy LLC against any actions brought by the minor participant against Norberto Ochoa Orejel and Ochoa Football Academy LLC. The parent(s)/guardian(s) also accepts responsibility for all medical expenses incurred by the participant.

All registrations and payments are final. No refunds, credits, transfers, or make-up sessions will be provided for missed sessions, schedule conflicts, illness, or non-attendance.
