Consent and Release: I, the undersigned parent or legal guardian, give permission for my child to participate in the waterslide and related activities at Davis Memorial UMC church event. I understand that participation in waterslide activities involves certain inherent risks, including but not limited to slips, falls, or other minor injuries. I voluntarily assume all such risks on behalf of my child.In consideration of my child being allowed to participate, I agree to release, waive, and hold harmless Davis Memorial UMC, its staff, volunteers, and representatives from any and all liability for injuries or damages arising from participation in this activity. I understand that there will be volunteers, supervising and monitoring the waterslide and give permission for the volunteers to remove my child from this activity if the behavior could cause harm to themselves or others.
