  I hereby grant my child/children permission to participate in Vacation Bible School (VBS) Program. This activity will occur under the guidance and direction of volunteers from St. Alphonsa Syro Malabar Church Baltimore on the above dates. As a parent, I remain legally responsible for any personal actions regarding the above-named minor(s). I hereby agree that no liability is assumed by St. Alphonsa Syro Malabar Parish or its volunteers for any claims, damages, or injuries which may arise out of this activity.
  
  Media Waiver:
  
  By signing this agreement, I hereby grant permission to St. Alphonsa Syro-Malabar Catholic Church of Baltimore, Maryland, the St. Alphonsa Vacation Bible School (VBS) Program, and its affiliates, coordinators, and volunteers to photograph, record, or capture my child(ren)’s likeness through photographs, video recordings, or other media during the VBS Program held on August 13, 14, and 15, 2026.
  
  I further authorize the use, reproduction, and publication of such media solely for promotional, informational, and program-related purposes, including but not limited to photos, videos, and social media content posted on the official St. Alphonsa Instagram account and other official parish communication platforms. I understand that no compensation will be provided for the use of such media, and that all media will remain the property of St. Alphonsa Syro-Malabar Catholic Church and its affiliated VBS program.
  
  Personal Injury/Security Waiver:
  
  I hereby discharge St. Alphonsa Syro Malabar Catholic Church of Baltimore MD, the St Alphonsa Vacation Bible School Program (VBS), and the affiliates/volunteers of the respective program of ANY liability in the case that my child (children) is (are) infected with any illnesses or are injured in any way.
  
  For the safety and protection of all participants, only authorized VBS personnel, volunteers, and approved church staff will be permitted on church premises during Vacation Bible School hours. Children enrolled in the VBS program will only be released to their parent(s) or legal guardian(s) at the conclusion of each program day, unless alternative transportation or pickup arrangements have been submitted in advance and expressly approved by VBS leadership. Authorized pickup individuals may be required to provide identification upon request.
  
  I agree that in the event that my child or children are injured/ill, I voluntarily agree to accept full responsibility of the issue, even if arising from negligence or omissions of St. Alphonsa Syro Malabar Catholic Church of Baltimore, MD or of its affiliated individuals.
