I, the undersigned, in consideration for the opportunity to participate in the Project Safe Neighborhood Athletic Combine ("Event") organized by the Lee County Sheriff's Office and held on property owned by the Ft. Madison Community School District, do hereby agree to the following:

1. Acknowledgement of Risk: I understand that participation in physical athletic activities involves inherent risks, including but not limited to falls, contact with other participants, weather-related hazards, and the risk of serious personal injury. I knowingly and freely assume all

2. Medical Authorization: I hereby give permission to the event organizers to seek emergency medical treatment on my behalf if I am injured or otherwise become ill during the Event. I understand I am responsible for any resulting medical expenses.

3. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, waive, and discharge the Lee County Sheriff's Office, the Ft. Madison Community School District, their officers, agents, employees, and volunteers from any and all liability, claims, demands, losses, or damages caused or alleged to be caused in whole or in part by the negligence of the released parties

4. Photographic Release: I grant permission for photos and videos taken of me during the Event to be used for promotional purposes by the Lee County Sheriff's Office.

5. Property Use Acknowledgement: I understand this event is being held on school district property and agree to respect all school rules, regulations, and facilities. I will conduct myself in a safe and respectful manner at all times.

I have carefully read and fully understand all the provisions of this Waiver and Release of Liability. By entering my full name below, I am freely, knowingly, and voluntarily entering into this Waiver and Release.
