Acknowledgment of Risk
I understand that participation in a youth football camp involves physical activity,
including running, jumping, tackling drills, and other athletic movements. I acknowledge
that these activities carry inherent risks, including but not limited to:
 Sprains, strains, fractures, and other injuries
 Heat-related illness
 Collisions with other participants
 Accidental contact with equipment
 Serious injury or, in rare cases, death
I voluntarily assume all risks associated with my child’s participation in this camp.

Medical Authorization
I certify that my child is physically able to participate in football-related activities. I
authorize camp staff, coaches, volunteers, or medical personnel to provide basic first
aid or seek emergency medical treatment if necessary. I understand that I am
responsible for all medical costs incurred.

Release of Liability
In consideration for my child’s participation, I hereby release, waive, and discharge the
following parties from any and all liability, claims, demands, or causes of action arising
from participation in the camp:
 Camp organizers
 Coaches and volunteers
 Facility owners
 Sponsors and partners
Photo & Media Consent
I grant permission for my child to be photographed or recorded during camp activities. I understand that these images may be used for promotional or educational purposes without compensation. If you do NOT consent to photos, please initial below:

Code of Conduct
I understand that all participants must follow camp rules, respect coaches and peers,
and maintain safe behavior. Failure to comply may result in removal from the camp.
Hold Harmless

I agree, by signing this form, to indemnify and hold harmless Le Guerrier EnterpriseLLC, the City of Ocoee, its officials, officers and employees and the Recreation
Department, its officials, officers and employees from and against any and all claims,
actions, causes of action, loss, damage, injury, liability, cost or expense, including
without limitation attorneys’ fees arising out of, resulting from, or occasioned by
participation in any activity associated with the City of Ocoee Parks & Leisure Services.

This release applies to injuries, damages, or losses caused by negligence or otherwise,
to the fullest extent permitted by law.

By signing below, I acknowledge that I have read, understood, and agree to the terms of
