Liability Waiver
By signing this waiver, I acknowledge and agree that I am aware of the risks involved in athletic activities and release Vega Stars Elite and its staff from any liability for injuries or damages that may occur during the 2026-2027 season.
Medical Release
I authorize Vega Stars Elite staff to seek medical attention for my child in case of emergency and agree to be responsible for any costs incurred. I confirm that my child is physically fit to participate.
Photo/Video Release
I grant permission for Vega Stars Elite to use photographs and videos of my child for promotional purposes, including social media and website content, for the 2026-2027 season.
