In consideration of name of contestant being permitted to enter
the above stated rodeo as a hometown or regular contestant, they, and the person who
signs as his or her parent or guardian, if any, accept full responsibility for and
hereby release and agree to save harmless of the HRA, all producers and
committees for and from all losses, injuries and damage that the applicant
may suffer or receive and also agree to indemnity the HRA, all producers and
committees from all losses or damage they may incur as a result of the
applicant participating in the rodeo.
I hereby confirm that I am 18 years of age or older or have parental consent
to enter the rodeo. I, name the parent or guardian along with the applicant, hereby agree to all of the terms as stated above.
