WAIVER & RELEASE OF LIABILITY:
By registering for and/or participating in the basketball camp hosted by Danny Green at Parkland High School , I (and/or the parent or legal guardian if the participant is under 18) acknowledge that participation in athletic activities involves inherent risks, including serious injury, illness, or death, and I voluntarily assume all such risks, whether known or unknown and whether arising from negligence or otherwise. I hereby release, waive, and hold harmless Danny Green, camp organizers, coaches, staff, volunteers, Parkland High School, sponsors, and all affiliated parties from any and all claims, liabilities, damages, or causes of action arising out of or related to participation in the camp, and agree to indemnify and hold them harmless to the fullest extent permitted by law. I authorize emergency medical treatment if necessary and accept full responsibility for any related costs. I also grant permission for the use of the participant’s name, image, and likeness in photos or videos for promotional purposes without compensation. By participating, I confirm that I have read and understand this waiver and agree to its terms voluntarily.
