    Note: We will not have VBS on Thursday, July 30 due to our mid-week church service at 7 PM.

  • Calvary Chapel Fellowship Church documents our VBS week to share the joy of the ministry with our church community. By consenting below, you grant the church permission to take photographs and/or videos of your child during VBS 2026. These materials may be used in church publications, worship presentations, the official website, and secure church social media channels. For your child's safety, no names or personal identifying details will ever be published alongside media.

    We completely respect your privacy. However, because our VBS program heavily integrates daily group photography, large-screen video recaps, and live media elements throughout the week, it is impossible for our volunteer staff to isolate individual children from background media. Therefore, this release is a mandatory requirement for attendance. Because you selected "NO," you will not be able to proceed and your child cannot serve at this time.

  • By completing the fields below, you acknowledge and agree that you are the parent or legal guardian of the minor child named above. You give full permission for your child to serve as a volunteer during the Vacation Bible School (VBS) 2026 hosted by Calvary Chapel Fellowship Church from July 27 to July 31, 2026. You understand the schedule includes early drop-offs (5 PM on Monday, 5:10 PM Tuesday–Friday) and pickups at 8:45 PM, with no VBS on Thursday, July 30.

    You understand that while your child is serving, the church will take reasonable safety precautions. However, you agree to assume all risks associated with your child's participation. You hereby release, waive, and hold harmless Calvary Chapel Fellowship Church, its pastor, officers, volunteers, and leaders from any and all liability, claims, or demands for personal injury, sickness, or property damage and expenses, of any nature, occurring during this event.

    In the event of an emergency, you authorize church staff or volunteers to administer first aid and secure professional medical treatment, including hospitalization or surgery, if you cannot be reached. You agree to assume all costs associated with such emergency medical treatment.

  • By signing below, you agree to volunteer your services for the Vacation Bible School (VBS) event at Calvary Chapel Fellowship Church from July 27 to July 31, 2026. You agree to follow all church policies, safety guidelines, and leadership directions during your time of service.

    You assume full responsibility for any risks of personal injury, illness, or property damage that may occur while you are serving on the church premises or participating in VBS activities. You hereby release, waive, and forever discharge Calvary Chapel Fellowship Church, its pastor, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action.

    In the event that you suffer a medical emergency while serving and are unable to communicate or make decisions, you authorize Calvary Chapel Fellowship Church leadership to seek and consent to necessary medical treatment or hospitalization on your behalf. You agree to assume all financial responsibility for any medical expenses incurred.
