This online Green Form consists of three parts.

Part 1 Club Section.

The secretary completes this part and by using the "submit form" button, sends this to the Demonstrator/Judge/Teacher/Speaker. On the email send to the Demonstrator/Judge/Teacher/Speaker, will be an invitation to open up their part 2 of the form.

Part 2 Demonstrator/Judge/Teacher/Speaker Section

The Demonstrator/Judge/Teacher/Speaker will receive an email containing a link to open their part of the Green Form.

The Demonstrator/Judge/Teacher/Speaker, completes Part 2 and by using the "submit form" button, sends this part to the secretary. The secretary will receive an email with an invitation to open the Part 3 of the form.

Part 3 Club Booking Confirmation

The club's secretary will receive an email with an invitation to open part 3 of the form to confirm the booking to the Demonstrator/Judge/Teacher/Speaker. This completes the cycle of the Green Form

Both the secretary and the Demonstrator/Judge/Teacher/Speaker will receive emails with details of the forms filled out and submitted.
