    CONSENT & AGREEMENT By submitting this form you confirm that: - Your child is joining a virtual group class (max 5 students)  - You agree to keep your contact information up to date and inform us of any absences Payment & Booking After submitting, you'll be sent a secure Stripe payment link. Your place is only confirmed once payment is completed. £95/month per child — cancel any time with 30 days notice before the 1st of the month. 👉 If you're enrolling more than one child, please submit this form separately for each child. You'll then be able to add sibling memberships (£85/month each) at checkout.  TERMS OF MEMBERSHIP — Please read carefully before submitting 1. Membership & Payments Membership is £95/month per child. A sibling discount of 10% is applied automatically at checkout. Payments are taken monthly via Stripe. Membership covers your child's place for the month — it is not based on the number of sessions attended. 2. Cancellations & Notice You may cancel at any time with 30 days notice before the 1st of the month. Payments already made cannot be refunded once the month has begun, except in exceptional circumstances at Urdu Club's discretion. 3. Missed Sessions Refunds or credits cannot be issued for missed sessions. Please inform us of absences so we can plan appropriately. 4. Communication Please keep your contact details up to date. Email is our main form of communication for updates, changes, and session reminders. 5. Photos & Videos We may use photos or videos for promotional or educational purposes unless you opt out above. 6. Behaviour & Wellbeing We aim to create a positive, inclusive and culturally respectful environment.
