FLIGHT LEAGUE PARTICIPATION WAIVER & RELEASE
I understand that participation in Flight League and aerial/circus activities involves physical activity including, but not limited to, climbing, spinning, balancing, conditioning, strength training, and use of aerial apparatus. I understand that these activities carry inherent risks including falls, strains, bruises, sprains, or other injuries.

I certify that the participant is physically able to participate in these activities and voluntarily assumes all risks associated with participation.

I agree to release and hold harmless Florida Aerial Dance & Circus Arts, LLC, its owners, instructors, staff, assistants, and volunteers from any and all claims, liability, damages, or injuries arising from participation in Flight League activities, except in cases of gross negligence.

I understand that participants are expected to follow all safety instructions and studio rules. Failure to do so may result in removal from activities.

I grant permission for photographs and/or videos taken during classes to be used for studio promotional and social media purposes unless otherwise requested in writing.
