Liability Waiver & Release
I, the undersigned parent/guardian, understand that participation in the Summit Football Youth Camp includes physical activity and athletic participation that may involve risk of injury.

I acknowledge that I am voluntarily allowing my child to participate in this camp and assume all risks associated with participation, including but not limited to falls, contact with other participants, equipment-related injuries, and other risks that may arise during athletic activities.

I release and hold harmless Summit Football, camp staff, coaches, volunteers, FCA representatives, and any affiliated organizations or facilities, including Latture Field and the Town of Abingdon, from any and all liability, claims, demands, or causes of action arising from injury, illness, or damages related to participation in the camp.

I certify that my child is physically able to participate in camp activities.

Photo & Video Release
