3:25 PM Meet at the Church to Carpool (only if your youth needs a ride)

3:45 PM Arrive at Boda Board Escape Room - 90 Pleasant St in Malden

6:15 PM Dinner & Paper Plate Awards at Sante Fe Burrito Grill - 205 Pleasant St in Malden

7:00 PM Dessert at Tous Les Jours - 230 Pleasant St in Malden

7:45/8:00 PM Arrive back at Church for pick up

FCCW is covering the cost of the escape room and dinner. Youth are responsible for the cost of dessert.

Sneakers or similar shoes are required, and we suggest you wear comfortable clothes. You will only be allowed to Quest with appropriate footwear. In particular, you cannot Quest in opened toe shoes, flip flops, sandals, heels, clogs, etc.

All participants must have a Youth Information Form on file with FCCW. If your youth does not have one on file, please complete the separate form which contains important medical information.

I understand that every effort will be made to contact parents/guardians in the event of an emergency. In an emergency requiring medical care and treatment, I authorize any physician, hospital or other healthcare provider to give care to this minor. I also give permission for the transport of my child/youth to/from a doctor and/or hospital by a chaperone or ambulance.

I hereby agree to hold harmless the First Congregational Church in Winchester and its agents and employees from any claim for personal injury or loss of life or property arising from participation in this Event. While I understand that the First Congregational Church in Winchester will take every precaution for safety, I realize that accidents do sometimes happen.
