I voluntarily assume all risks related to my child’s participation and release, waive, and hold harmless the organization, its officers, coaches, staff, affiliates, sponsors, and representatives from any liability for injuries or damages arising from participation in camp activities, including travel to and from camp.

In the event of injury or emergency, I authorize necessary medical treatment for my child and understand that I am responsible for any related expenses.
