This tattoo inquiry form is to help Christian stay up to date with his appointments and all appointment requests as he stays busy! Deposits in person with Christian are still required for the appointment to be set! If you aren’t local(45 or less) deposits may be sent to the shop owner(Kevin) via cashapp to save your travel! All deposits are subtracted from the price of the overall tattoo! YOU MUST be 18 years or older to get tattooed. Christian will not, by any means, tattoo you until you have filled out the shop ID/tattoo form at the shop, thank you!
