MEDIA & PROMOTIONAL CONSENT: Unity Under the Hood may photograph, record, livestream, interview, or otherwise capture portions of the event for promotional, recap, marketing, and media purposes. By submitting this form, I acknowledge that, if selected as a panelist, my image, voice, name, professional title, and event participation may be captured and used in Unity Under the Hood and CF Productions promotional content related to the event.

By submitting this form, I understand that: Submission of this form does not automatically confirm panelist placement unless otherwise mentioned. Final panelists will be selected based on event needs, expertise, topic alignment, and overall panel balance. Selected panelists will receive official confirmation and additional event details from the Unity Under the Hood team. I agree to communicate promptly regarding my availability and participation. If selected, I will notify the event team as soon as possible of any changes to my availability.
