1. Permission to Use: I allow the Church of Scientology of the Valley ("Valley Org") to record and use my child's name, voice, and image for the Project / Purpose of Use listed above. This includes films, photos, digital media, and advertisements. They can use these materials forever, anywhere in the world, and in any format (online, print, etc.).

2. Ownership & Rights: I give the Valley Org full ownership of any recordings or photos made for this project. They have the right to edit, copy, sell, or share these materials however they see fit. I agree that I do not need to inspect or approve the final product before it is used. I also agree to keep confidential all matters relating to the recording(s), including my participation in the same, until such time as the recording(s) have been released to the public.

3. No Obligation: The Valley Org is not required to use my child's image just because I am signing this form; it is at their discretion.

4. Liability Release: I agree not to sue the organization or its staff, representatives or assigns for any reason related to this project. This includes claims for privacy violations, defamation, or copyright issues. This agreement is final and applies to me and my heirs.

5. Legal Authority: I confirm that I am the parent or legal guardian of the child named above and have the authority to sign this agreement.
