By booking and participating in mobile massage therapy services provided by Tracie Weber Licensed Massage Therapist), I acknowledge and agree to the following:
I understand that massage therapy is intended for relaxation, stress reduction, muscle relief, and overall wellness purposes only and is not a substitute for medical care, diagnosis, or treatment.
I confirm that I have disclosed all relevant medical conditions, injuries, allergies, sensitivities, recent surgeries, pregnancies, or health concerns that may affect my session.
I understand that it is my responsibility to inform the Therapist of any discomfort, pain, or concerns during the session so adjustments may be made for safety and comfort.
I acknowledge that massage therapy involves physical touch and manipulation of muscles and soft tissues and may carry certain risks including soreness, bruising, skin sensitivity, aggravation of pre-existing conditions, or other unforeseen reactions.
I voluntarily assume all risks associated with receiving massage therapy services and release, waive, and discharge Massage With Trace, Tracie, and any affiliates or assistants from any and all liability, claims, injuries, damages, losses, or expenses arising from participation in these services.
I understand that Massage With Trace reserves the right to refuse or discontinue services at any time for safety, health, inappropriate behavior, or boundary violations.
I acknowledge that all massage services provided are strictly professional and therapeutic in nature. Any inappropriate conduct, solicitation, or behavior will result in immediate termination of the session without refund.
For mobile appointments, I agree to provide a safe, clean, respectful, smoke-free, and appropriate environment for services to be performed.
I understand that cancellations or rescheduling require a minimum of 2 hours’ notice. Late cancellations or no-shows may be subject to fees.
I voluntarily consent to receive massage therapy services provided by Massage With Trace.
