  • Welcome to my booking form!

    My name is Abbey! I’m a black & grey tattoo artist working at Witch in the Woods Tattoos located in Larder Lake, Ontario. Thank you for coming along to fill out my submission form! This form can be used to book tattoo appointments as well as consultations. Very excited to hear about your ideas and can’t wait to work with you!

  • The purpose of this form is to get a clear concise idea of what you're looking to have tattooed so please answer all questions and fields carefully and thoroughly! Once I have read through your submission I will email you, or contact you with the rest of the details.

    Some IMPORTANT information before you begin:

     

    ✦ My minimum price per piece is $120+HST

    ✦ My hourly rate is $170+HST

    ✦ Pre Drawn Flash has flat rate & sizes (pre drawn flash are designs I have created from my brain that I think are fun pieces that I'd love to tattoo, they can be found on my instagram page under the flash highlight)

    ✦ A non-refundable deposit of $100-$150+HST depending on the piece will be required to book a tattoo appointment. Please do not send any money until you are asked for a desposit and please do not pay more than the deposit amount unless previously discussed!

    ✦ My working dates are Wednesday through Saturday (with the occasional off day available).

    ✦ You may submit multiple ideas in one form, just please keep them as organized as possible. If you feel they need a separate form, fill out another :)

    ✦ Please allow one to two week to hear back from me as I am only one small human who's working doing it all and my brain hurts sometimes. Even if I can't take on your piece, I will still contact you to let you know! Thank you for your patience!

     

    Restrictions:

    I will not tattoo anyone under the age of 18, no exceptions.

    I will not tattoo anyone who is pregnant.

  • Custom Tattoo: an original one of a kind design that I create specifically for you based on the information given in this form or discussed during consultations or other communications. I will not copy another artists work, all designs are uniquely created by me. (Also I know AI is everywhere, please try to refrain from using Ai generated reference photos where possible, let me be your ai generator ok, I guarantee I drink less water lol)

    Pre Drawn Flash: a series of pre drawn designs at a reduced rate that I've drawn up because I would love to tattoo them. To find my available flash designs please see the flash highlight located on my instagram!

  • Deposits

    The deposit works as a placeholder for your appointment date. Your appointment is not confirmed until the deposit is received. Deposits should be received within 24hrs to secure your appointment. Deposits are not required for consultations (consultations are free)
