The Horizon Award for Early Career Excellence recognizes an outstanding early career family physician who is already shaping the future of family medicine through exceptional leadership, dedication, and innovation. This prestigious award honors a physician who, within their first seven years of post-residency practice, has consistently demonstrated a commitment to excellence in patient care, community health, and professional service. Note: Nominations may be submitted by any individual familiar with the nominee’s contributions. Preference will be given to nominations from colleagues who work closely with the physician in a day-to-day capacity.
