This form is to schedule a loan ONLY when you have been accepted into our program. We must have your information in our system for this request to be processed. Please understand, not all requests can be fulfilled.

Our programs require a clear Motor Vehicle Record on all drivers. Please complete the information for any person who will be driving the vehicle during this loan.
I authorize DriveShop to obtain a current Motor Vehicle Record of my driving history now or any time in the future that I have access to a DriveShop vehicle. I agree that I will not be permitted to operate or have access to a vehicle if my driving record indicates driver ineligibility, as determined by each client and program. All drivers are required to complete DriveShop's Borrowed Vehicle Agreement, to be sent separately upon confirmation.

Vehicle Loan Request Information

DriveShop offices deliver vehicles Monday through Friday during standard business hours (9am-5pm). Standard vehicle loans are seven days. Loans longer that the standard duration may require client approval and additional content deliverables.
