PARTICIPATION WAIVER & RELEASE OF LIABILITY
I, the undersigned parent or legal guardian of the participant listed in this registration form, acknowledge and understand that participation in the Elite Performance Audition Intensive and related movement programming involves physical activity and inherent risks, including but not limited to falls, collisions, sprains, fractures, illness, and other potential injuries associated with dance, tumbling, acrobatics, conditioning, flexibility training, and athletic movement activities. I certify that the participant is physically able to participate in these activities and agree to disclose any relevant medical conditions, injuries, allergies, or physical limitations that may affect participation. I understand that Amanda Marie Training will take reasonable precautions to promote a safe training environment; however, I voluntarily assume all risks associated with participation. In consideration for participation in this program, I hereby release, waive, and discharge Amanda Marie Training, Amanda Bernett, studio rental facilities, assistants, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from injury, illness, loss, or damages related to participation in the program, except in cases of gross negligence or willful misconduct.
The following selection serves as my agreement/signature for Liability Waiver acknowledgement.
EMERGENCY MEDICAL AUTHORIZATION
In the event of illness or injury requiring emergency medical treatment, I authorize Amanda Marie Training and its representatives to obtain emergency medical care for my child if I cannot be reached immediately. I understand that I am financially responsible for any resulting medical expenses.
The following selection serves as my agreement/signature for Emergency Medical Authorization.

TUITION & REFUND POLICY AGREEMENT
Full payment of $720 is required to reserve a participant's spot.

Cancellations received on or before July 12, 2026, will receive a refund of $620. A $100 non-refundable administrative deposit will be retained.

Cancellations after July 12, 2026, are non-refundable due to limited enrollment and facility commitments.

If Amanda Marie Training cancels the program, all tuition paid will be refunded in full. In the event that Amanda Marie Training must cancel a session, a makeup class or alternative instructional opportunity will be provided whenever possible.

Attendance is strongly encouraged due to the progressive nature of the intensive curriculum. Makeup classes are not available for missed sessions. Tuition is not prorated or refunded for participant absences.

As a benefit of participation, intensive athletes are eligible for a 10% discount on up to two private coaching sessions scheduled within 60 days following the intensive. Private sessions may be used for individualized technique training, audition preparation, flexibility development, tumbling progressions, choreography support, or performance coaching.
