PLEASE READ BEFORE SUBMITTING! I am currently booking for September 2026. Thank you for your interest in booking with me! This form will allow me to provide you with the most accurate quote. Please be as detailed as possible when filling out this form!! Once the form is complete, I will reach out via Instagram DM (or text if no Instagram), and provide you with a quote and the link to book online. At this time, I will also finalize any details and answer any questions you have. Please keep in mind that I only tattoo in a fine line style, with black/grey and red ink. I do not perform cover ups.
