PERSONAL DATA & PRIVACY INFORMATION
Personal data provided in this application form will be used for assessing your eligibility for admission to the eBRAM List of Pro-Bono Mediators and for purposes incidental to it. Data herein will be dealt with by eBRAM staff and/or by the eBRAM Appointment Committee or other relevant eBRAM Board members. After an application has been processed, all relevant documents will be retained in a file opened by eBRAM for each applicant. Such information will be retained by eBRAM for up to 10 years from the date on which this Application Form is received, the date on which the applicant was last appointed as Pro-Bono Mediator, or the date on which we last received from you in writing your intention to remain on the eBRAM List of Pro-Bono Mediators (as applicable), whichever is later. You may at any time make a request to us in writing to have all (or, if feasible, part) of your information held with eBRAM removed to the extent practicable. Under the provisions of the Personal Data (Privacy) Ordinance, you have the right to request access to, and the correction of, your personal data as retained by eBRAM. Such requests under the Personal Data (Privacy) Ordinance should be submitted in writing to the Data Protection Officer at eBRAM, Room 403, 4/F, West Wing, Justice Place, 11 Ice House Street, Central, Hong Kong, or by sending an email to dpo@ebram.org.
By submitting this form, I do solemnly and sincerely decare as follows:
1. I have read, accept and acknowledge the Personal Data & Privacy Information statement above.
2. I authorise eBRAM, its staff, employees and/or members of eBRAM or other relevant eBRAM Board members to deal with, utilize and/or assess the personal and other data submitted by me in connection with my application.
3. I declare that the information contained in this Application Form is true, accurate and complete to the best of my knowledge. I understand that if any information contained in this Application Form is untrue, inaccurate and/or incomplete, my application may be disqualified and my admission to the eBRAM List of Pro-Bono Mediators may be revoked.
4. I confirm that I have not been found guilty by a Court or disciplinary tribunal of misconduct.
5. I confirm that I will abide by the Hong Kong Mediation Code, should my application be accepted.
6. I confirm that I shall be bound by eBRAM disciplinary processes should a complaint be made against me. I also confirm that I shall be bound by any sanctions imposed by eBRAM’s Challenges Committee, Panel Listing Committee and/or Board of Directors, which may include my removal from the eBRAM List of Pro-Bono Mediators.
7. I agree to submit my CV and other additional supporting documents attached herewith for the purpose of the assessment of my application.
8. I acknowledge and confirm that my role as a mediator (if any) in connection with any pro-bono mediation schemes devised by eBRAM is voluntary and I will not receive any compensation or remuneration for my attendances.
9. I consent to eBRAM releasing information relating to my involvement in any pro-bono mediation schemes devised by eBRAM to third-party organisations for their recognition of my voluntary involvement in such schemes.
