This award recognises the transformative impact facial aesthetic treatments can have on patients’ lives. It celebrates the most inspiring patient journey—highlighting meaningful change, both aesthetically and emotionally.

Things to note before entering:

• You must hold a valid 2026 IAAFA Conference ticket for your application to be considered.

• Please complete all sections of this application form, ensuring that you have all the information required and are applying for the correct category. If you wish to save your application form as a draft and return to it at a later date, you can do so by clicking the "save" button at the bottom of the page (saves for 3 months). 

• All applications must include before and after pictures. Winning case studies may be shown along with the winners announcement on the night. Please ensure you have permission from the patient to use their pictures in this way. IAAFA cannot be held responsible for the publication of case studies where full consent has not been met.

• You are only able to enter the same case study into a maximum of two different award categories.
