Lace Up for Kids Back to School Shoes Application

All school-age children from low-income families, grades K-12, in Hancock, Brooke, Ohio, Marshall, Wetzel, Tyler Counties in WV, and Belmont County, OH, are eligible to receive a voucher that can be redeemed at several Shoe Department locations toward shoes just in time for the new school year! Please note the child(ren) will NOT need to be present for voucher pick up but WILL need to be present to redeem the voucher in store.

Valid and working email is required as all communication about applications and pick ups will be through email.

Parents/Guardians will need to show proof of ID and address at time of pick up.

You are responsible for transportation to and from the distribution dates. If you are unable to make any of dates listed, or cannot send an alternate pick up person, you may make an alternate pick up appointment at the United Way of the Upper Ohio Valley office in Wheeling. No exceptions will be made.

You are permitted to pick up at ANY of the county distributions and are not limited to only your county of residence.

Do you or will you have a mode of transportation to make it to your county's distribution day? We are unable to accommodate any drop offs this year. If the date for your county does not work for you, you can select another date that does or select alternate pick up.
We may take photos during distributions to share on United Way UOV social media, website, and other program-related marketing materials. Families who prefer not to be photographed will have the opportunity to attend during a designated no-media portion of the distribution. Please acknowledge below that you understand this policy.
