Only the person(s) who received the Sacrament, with a current valid photo ID, may submit a request. A parent/legal guardian can request the certificate if it is for a minor. All certificates are provided in hard copy with the church seal. Certificates may be sent via email directly to another church only. Please allow up to 10 business days to research and provide a hard copy sacrament certificate. Please note that a Baptism certificate will include notations for any other sacraments that are noted in the record book.
